Section 1 Scope/Objective
Objective of the enhancement is to collect "Customer Satisfaction Survey Form" through online which are being collected offline from Departments.
Section 2 Current Process Flow (if any)
W.r.t. requirement "TW_REQ_E136_User Feedback and Customer Satisfaction Survey Form", presently when user choose TEMPLATE 2 flag for feature "Is Feedback and service Rating required", pop up message displays for all departments users/buyers.
Section 3 Proposed Changes/New Requirement details
3.1 New field details (if any)
New value for existing feature "Is Feedback and service Rating required" is Yes (Buyer and Bidder)
3.2 Detailed description of Proposed Changes/New Requirement details
- For the exiting feature "Is Feedback and service Rating required" incorporate a new value for the dropdown selection as "Yes (Buyer and Bidder)".
Note: Bidder side flow yet to share by Mobility Team

When feature "Is Feedback and service Rating required" is set as "Yes (Buyer and Bidder)" then, provide a new menu as "Customer Satisfaction Survey Form" under Assign Menus and should be assignable for ADMIN and ADMIN Users of Centralized and Decentralized sites.
For Buyer after successful login, provide a new link as "Customer Satisfaction Survey Form" under left menu only when user assigned with new menu "Customer Satisfaction Survey Form".
Note: Bidder side flow yet to share by Mobility Team For Bidder after successful login, provide a new link as "Customer Satisfaction Survey Form" under left menu only when feature "Is Feedback and service Rating required" is set as "Yes (Buyer and Bidder)".
A new "Superuser" should be provided. "Superuser" should be allowed to create Survey Form Formats. Created templates should be allowed to copy to all the TW and AW departments. For the selection of departments checkboxes to be provided with Select All option.
Admin End Process: a. When Admin clicks on the link "Customer Satisfaction Survey Form" under left menu below page should be loaded to create User Feedback questionnaires.
b. By clicking the "+" button in above figure, below fields should be loaded and user should be allowed to create the User Feedback Header:
c. Below listed header fields and its actions should be added to create a new feedback header in above figure:
Title: User feedback form Title
Description: Enter the description for the new form
Valid From: From when user should get option to enter the answers
Valid To: Till when user should be allowed to enter the answer
Other Fields: Additional fields should be added using this option
Show Group Chat: When this option is checked then Users should get option to Chat for this Survey Form.
d. Provide two links to create an individual Question and Group Questionnaire: - + Add Question
e. When user clicks on "[+ Add Question]{.underline}" link, below fields should be loaded to create questionnaire and fields inputs of below image are:
Question: User can enter the question.
Answer Type: Below listed answer types should be provided

Rating with Heart Icons
Rating with Circle Icons
Text Area
Radio Button
Check Box
Blocks
Scale Slider
Scale Slider With Icon
Answer: This field will be used by the Buyer and Bidder to enter their answer.
Placeholder value is "Please Enter Answer".
Required: Mandatory or Not Mandatory
Show Comments: Additional field should be provided to provide any comments with respect to questions
f. To create questionnaires group "[+ Add Group]{.underline}" link should be used. By clicking the said link below group should be created. Inside the group users should be allowed to create questions by clicking "[+ Add question in group]{.underline}".
g. By clicking the "+ Add question in group" below listed fields should be loaded and fields inputs of below image are:
- Question: User can enter the question.
- Link: URL
- Answer Type: Dropdown
- Answer: This field used by the Buyer and Bidder to enter their answer.
- Placeholder value is "Please Enter Answer".
- Answer Values: For Radio Button/Check Box this field should be
provided with below field values/actions:
- Label: Text field
- Weightage: Marks for the fields which will be selected by the user
- Is Default: Default value can be set
- Required: Mandatory or Not Mandatory
- Show Comments: Additional field should be provided to provide any comments with respect to questions
h. When user clicks on "[+ Add Link]{.underline}" below fields should be loaded should be allowed mentioned inputs: - URL: Web site URL link - Name: Enter the name for the above link
i. Provide below pop up when user clicks on "SAVE" button:
j. Provide below pop up when user clicks on "SUBMIT" button: Note: Once submitted user should not be allowed to edit any inserted fields in the created Feedback Form.
k.Submitted form should be displayed as below and user should not be allowed to re-edit for the submitted form.
Icons Description:
- View Questionnaires
- All Users who has given feedback
- Individual User Feedback
l. When Admin clicks on Individual User Feedback icon, user who responded to questionnaires it should be displayed as below.
m. When Admin clicks on Chat icon, below comment screen should load. - Clarity required on which will capture, User ID or User Name displays by clicking send button - Placeholder value is "Type a message".
Buyer End Process:
a. When Buyer clicks on the link "Customer Satisfaction Survey Form" under left menu, below page should load. Where, created forms should display:
-  - This icon will be used forchatting purpose-  - To answer thequestionnaire this icon will be used-  - All Users who has givenfeedback
b. By clicking edit icon below Admin created Form should load:
c. Provide below pop up when user clicks on "SAVE" button:
d. Provide below pop up when user clicks on "SUBMIT" button:
Note: Once submitted user should not be allowed to edit any inserted fields in the created Feedback Form.
When user clicks Graph icon below page should load:
- Existing functionalities of TEMPLATE 1 and TEMPLATE 2 flag in feature "Is Feedback and service Rating required" should be retained.

3.5 Special Concerns
Ensure on retaining the existing functionalities.
Section 4 Acceptance criteria
Existing functionalities of TEMPLATE 1 and TEMPLATE 2 flag in feature "Is Feedback and service Rating required" should be retained.
This new process applicable to only those Buyers who are assigned with new menu "Customer Satisfaction Survey Form".
